If you’ve had an issue with a food product, we’d like to hear about it.

Use this form to let us know what happened, and we’ll look into it.

Before you start, things you need to know:

• you must still have the product, or we will be unable to process this form
• your complaint will be reported to Environmental Health

Before you start, you will need to tell us:

• the address where you got the product from
• details of the product and the issue
• if you have informed the product retailer or manufacturer of the issue
