Your report helps us keep these structures safe and well-maintained for everyone.

This form is to be used for asking about or reporting damage of structures like:

• bridges
• culverts (what is a culvert?)
• subways
• footbridges
• sign gantries (what is a sign gantry?)
• retaining walls

Before you start, it would help if you told us:

• the type of structure you are asking about
• if the structure is damaged, details of the damage, and who caused it (if known)
