Before you start, things you need to know:

By completing this form, you agree to the following terms and conditions.

• you agree to return the Direct Debit Form authorising payment of the fee
• you agree to make sure all payments are paid within 6 months of the start of playing season
• you understand that by submitting this request you are liable for any payments that are not made

Please make sure that your team is registered to play with your district league.

Please also make sure that you know all your team's organisation contact names, addresses, and the contact details of your league secretary. If a club has multiple teams, each team must submit a separate application for pitch hire.

Before you start, you will need to tell us:

• which pitch you would like to use
• the time and day you would like to use the pitch for both official matches and/or training purposes
• the sport you would like to use the pitch for
• details of your team (team name, age group, number of people, etc.)
• which league you will be playing in
• the name and phone number of the league secretary
